Medical Director (Primary Care) Opportunity next to Allentown, PA!!
Position Description
Essential Duties:
· Assumes overall accountability and responsibility for the medical care of the participants.
· Provides clinical oversight and ongoing supervision, mentorship and training of primary care physicians and nurse practitioners.
· Provides direct primary care for an appropriate panel size of participants.
· Performs interval assessments on new and established participants, as required by PACE regulations.
· Admits participants to hospital, SNF or other healthcare facility, providing primary care attending responsibilities and/or coordinates and manages care through proactive collaboration with hospitalists; provides updates to the Interdisciplinary Team (IDT) on a regular basis and, when required, assists with the coordination of discharge panning with the IDT.
· Conducts home visits, as required.
· Provides leadership and guidance to the IDT to achieve care plans goals, meet emergent and acute needs of participants, and assures comprehensive and continuous care.
· Ensures development and compliance with organizational policies and procedures and all applicable regulatory requirements and standards of practice.
· Co-chairs the Quality Assurance and Performance Improvement Committee (QAPI) and leads clinical process improvement activities to ensure the eLIFE programs attains the highest quality outcomes for its participates and satisfies all PACE regulatory requirements.
· In collaboration with the Director of Operations ensures compliance of the QAPI program with organization standards and regulatory requirements.
SALARY: $200,000+
60-80% Medical Director/20-40% PCP
