an employed physician you will have two separate opportunities at the end of each contract year to earn a financial bonus: (1) gross billings, AND (2) max of 7% of base income based on percentage of Medical Center's strategic values met..

The first bonus is based on your gross billings/charges annually. So it is purely based on your productivity and what billings you generate. We will pay you 25% of your gross billings/charges annually that are in excess of your business plan annual gross billing target. Cap at $200,000.

For example, if your annual business plan gross billing target is $400,000 (or $33,000 a month) and your actual gross billings at the end of 12 months is $480,000, then we will issue you a bonus for $28,000 in addition to your annual income. (Your excess billings over the target were $80,000 x 25% = $20,000)

There is also an opportunity for you to earn up to a maximum of 7% of your base income based on a percent of the Medical Center''s strategic values met. For bonus purposes, we measure safety, service, relationships, and quality (performance is not considered for bonus purposes).

An example: *Physician's salary of $200,000 - your maximum bonus potential is $14,000.
*Percentage of Medical Center's strategic values met = 75%. (avg. 70-80%) Percentage of physician's bus. plan met (Target $1.5M: Actual $1.0M) = 67%
*BONUS CALCULATION = $14,000. x 75% x 67% - $7,035.
