WHY THE POSITION IS OPEN: Physician leaving due to Medical reasons

START DATE (ESTIMATED): ASAP

PRACTICE SETTING: Satellite office for 30+ multi-specialty group. However this location consists of 2 FT FP's.

CALL ROTATION: 1 in 3 weeks

OFFICE HOURS: Sept through May - 7:30 - 4:30 first patient seen at 8 and last patient seen at 4:15 and June through August - 7:15 - 4 First patient seen 7:45 and last patient seen at 3:45. Docs work 4.5 days.

PATIENT POPULATION % (I.E. HMO, MEDICAID): Medicare ~31%, Medicaid ~4%, Commercial Insurances ~65%

Age Range~ Newborn to 102 years old ~ Average Age = 50.2 years old

ANTICIPATED PATIENT LOAD PER DAY/WEEK: Average 25-30 per day (4.5 days per week)

COMPENSATION:
SALARY RANGE: $Negotiable
BONUS AND INCENTIVES: 1st year guarantee, then directly onto our compensation methodology - Productivity less direct and an allocated portion of indirect expenses.
